Select the appropriate forms from the blue links below. Please do not mail them to us but bring them with you when you come to MTF. There are four forms. You may not need to sign them all.
Every visitor and every rider that enters the facility must sign the "Release Form". If the visitor or rider is under 18 they must sign and have notarized the "Release Form - Minor". This form should also be signed by a parent.
For parents of a minor participant (under age 18) that will be absent for any period of time during the camp you must complete, sign and have notarized one of the "Healthcare Surrogate Designation" forms.
Choose the "Healthcare Surrogate Designation (MTF) form if you wish MTF staff to make healthcare decisions for your minor child.
Choose the "Parental Consent/Healthcare Surrogate Designation (specified person)" form to have a friend or family member make health care decisions and sign any necessary forms with your consent. This person must be present at MTF for the duration of the entire camp.
All forms must be notarized. For parents traveling to MTF we have a notary available at the facility
